Annual Well Exams (Includes sports clearance)

Dear Parents and Families,

Did you know your child can complete their annual well exam and sports clearance in the same visit at our office?

Our annual well exams follow the guidelines of the American Academy of Pediatrics and include a comprehensive head-to-toe physical, growth and development assessment, immunization review, and preventive health screenings. If your child needs a sports form completed, we can take care of that during this visit as well.

Unlike a limited sports physical at a walk-in clinic, our well exam is a complete evaluation of your child’s overall health — all in one convenient appointment.
